The purpose of this Request for Proposal (“RFP” ) is for the Fort Belknap Indian Community Tribal Health Administration (FBICTHA) to enter into an agreements with professional service providers who are experienced in manufacturing of Mobile Dental Vehicles.

The Fort Belknap Indian Community Tribal Health Department is interested in receiving proposal for the purchase of a mobile dental vehicle with the following specification:
Chassis 2020 Diesel Engine, Unit length 40’ 0” with bathroom & (1) 20KW Diesel generator , 2 separate sound proof rooms equipped with 2 dental chairs , delivery systems, 2 dentist and 2 assistant stools , 2 wall mount radiographic machines. Sink in each room. No CDL required
The vehicle should not exceed the Gross Weight Capacity of 26,000 lbs, Leveling Stabilizing System (Quadra bigfoot). Door between Cab & Clinic , Winter Package , Tank Blankets & Spray Foam 40’, Fuel Capacity minimum 100 gallons, cruise control, cloth Seats , Power locks/windows, premium Stereo, Back up alarm and back Up Camera. AM/FM Stereo , Bluetooth/MP3 satellite compatible 30” Heavy Duty Side Entry Dorr w//Window , latch and Deadbolt Lock, minimum 3 windows; One (1) 80 Amp smart Charge technology Battery and Charger; One Shore Power Cord 50 foot 240V 50 A;1 Isolator –Heavy Duty 200 Am (charges auxiliary battery from vehicle alternator); LED interior lighting; 10-14 Electrical Outlets- 110V with USB Port; 12 Volt D.C. Breaker Panel; 120Volt A.C Breaker Panel; Six CAT 6 Data Outlets ; 3 Air conditioning; 3 wall mount heaters, and 1 toe kick heater; 1 LP Gas Heater 30,000 BTU with tank; water pump; water heater. City water Fill exterior hose connection. Carbon Monoxide, Smoke detectors. Refrigerator, 24” TV, Graphic Budget, Highway hazard Kit, Install Oxygen Tank cylinder and ultrasonic cleaner . Total of 3 sinks in the mobile vehicle with hands free faucet. Interior Cabinetry and countertops constructed with moisture resistant material . Locks in each drawer a cabinet. Air compressor for 2-3 user, Vacuum system for 5 users. Dedicated outlet and Structural Plating for Panorex installation in the reception area ; 3 glove box, 3 soap dispensers, 3 trifold paper towels dispensers 2 x-ray Apron Hanger, sterilizer. Warranties for frame 5 years/unlimited mileage ; transmission 4 years/unlimited mileage ; Chassis and Cab 2 years/unlimited mileage, chassis batteries 1 year or 100,000 miles.
The FBICTHA reserves the right to amend, withdraw, or cancel this RFP and reserves the right to reject all Proposals at any time prior to execution of the Agreement. The Fort Belknap Indian Community Tribal Health Administration reserves the right to request or obtain additional information about any and all Proposals.

A projected timeline outlining the major steps and completion date of the mobile dental vehicle.
