Connie Jacobs and her nine-year-old son were fatally shot by an RCMP Constable on the Tsuu T'ina reserve in March 1998. A report issued by a judge on Thursday called the incident a modern tragedy.

The report makes 18 recommendations to the Tsuu T'ina tribal police force and Tsuu T'ina Child and Family Services in order to prevent future incidents. Jacobs' six children were to be apprehended by social services the night she and her son were killed.
